Complete the form below to have the COVID-19 Vaccination Badge applied to your profile. This badge may be required to participate in certain activities with the Mazamas. When that is the case, if you do not have this badge you will be unable to register or apply for that particular program/activity.
If you are not comfortable uploading a copy of your vaccine card, you may stop by the MMC during open hours, Tuesday through Thursday from 10:30 a.m. – 4 p.m. and present your card. The badge will then be added to your profile.
